Complete this form and return it to JAVC at address above or send via Email to:
Admin@javc.org to reserve your table & spot as soon as possible. Event planner is Sarah Pleth, 928-
681-2589. Table money of $25 is to be collected no later than 30 days prior to event which would be October 2, 2024. We can accept your payment by credit card, check, or cash! Please be advised that this is non-refundable if you
don’t show. Please make checks payable to Jerry Ambrose Veterans Council or call in for a credit card
payment and speak to Shirley (Nani) Chrimes at 310-721-2521  (office). Cash payments will also be
received at JAVC Operation 6, 315 E Oak Street, Kingman. Please let the office know that you are paying
for a spot in the Holiday Bazaar if you do not pay at time of application. All applications will be given to
Event Organizer, Sarah Pleth.
Location for this event will be at: The Kingman Elks Lodge #468, 900 Gates Avenue,
Kingman, AZ – right across from the golf course.
Rules:
➢ All vendors must set up in the morning at 7:30 AM and be ready by 9:00 AM for customers. All
vendors must stay for the full duration: 9am-2pm.
➢ All vendors must check in with Sarah or Joe Pleth for table assignment.
➢ No extra side tables will be allowed due to space restrictions.
➢ Food will be available for purchase. No outside food is permitted.
➢ All set-ups to have a holiday theme; all items can be sold – not necessarily only Christmas items.
